Who needs the Submission Form for Food Safety Management System
This licence applies to Singapore businesses registered under the following SSIC industry codes:
Required documents and prerequisites
Items the applicant typically needs ready before submitting:
- FSMS Plan (for premises which are required to implement FSMS) (mandatory)
- Company information/ FSMS team/ Product description
- Flow diagram (with CCPs identified)
- HACCP charts for CCPs which includes:
- Identification of hazards at CCPs
- Control measures and critical limits
- Monitoring procedures and documentation
- Corrective action to be taken
- Acceptable File Format: GIF,JPG,PNG,PDF
- Maximum upload per application: 5MB
- Valid FSMS trained certificate accepted by SFA (mandatory)
- To submit the certificate of personnel who develops the FSMS plan.
- The personnel must be certified on any of the following courses:
- 1. WSQ Apply FSMS for Food Service Establishments
- 2. Food Safety and Hygiene Level 4 (Advanced FHO)
- 3. Lead Auditor course (IRCA certified) e.g. IRCA certified ISO22000 Lead Auditor Course
- Acceptable File Format: GIF, JPG, PDF, PNG
- Acceptable File Size : maximum 5 MB
- Accredited FSMS Certificate (mandatory)
- To submit an accredited certificate issued by 3rd party conformity assessment body (e.g. certification body) accredited by the Singapore Accreditation Council (SAC) or an internationally recognised accreditation body.
